By filling out this form, you agree to be locked into a minimum 6 months The Fitness Factory Cobargo or Bermagui gym membership. You agree for The Fitness Factory to direct debit your account on a fortnightly basis, structured around your membership type. You will be charged a one off $60 joining fee, direct debited along with your first 2 weeks membership. Follow this link to complete a pre-exercise health questionnaire and waiver, this will then lead you to a link to our GoCardless Direct Debit system.
